Once you schedule an appointment, you can save time by going to the
Forms page on this site to print the HIPAA Notice of Privacy Practices
and fill out the Application for Clinical Services packet. Everyone is
required to complete these forms, prior to the first session. You will be
asked to sign a form acknowledging that you have received the HIPAA
Notice of Privacy Practices. If you do not have a printer or if you forget to
bring the forms with you, then Dr. Bolton will give you a packet when you
arrive at your appointment.
