Services are By Appointment Only Monday - Thursday 9:30 am - 6:30 pm Friday 9:30 am - 4:00 pm Once you schedule an appointment, you can save time by going to the forms page on this website to print and fill out the Application for Clinical Services packet. Everyone is required to complete these forms, prior to the beginning of the first session. If you do not have a printer or if you forget to bring the forms with you, Dr. Bolton will give you a packet when you arrive at your appointment. |
